Clinical Governance
- Clinical governance is a structured framework aimed at ensuring an ongoing continuous
improvement of patient service and upholding the highest standards of care within a hospital system. Tailored primarily for investors, owners, directors, and managers of hospital and healthcare business enterprises, this specialized oversighting discipline revolves around various key aspects including the quality of patient services, patient and provider safety, organizational reputation, and financial sustainability. - At its core, clinical governance is focused on the effective governance and management of hospitals and healthcare enterprises, with a dual objective of delivering tangible business value and returns to investors while ensuring optimal value for patients. Through this program, stakeholders are equipped with the tools and strategies necessary to navigate the complexities of the healthcare industry, fostering a culture of continuous improvement and accountability.
The Evolution of Clinical Governance
- The roots of clinical governance trace back to the UK, following what came to be known as the notorious Bristol Heart Scandal of 1995. Dr. Stephen Bolson’s expose revealed alarming mortality rates in pediatric cardiac surgery at The Bristol Royal Infirmary, prompting the inception and definition of Clinical Governance within the UK National Health Service later that same year.
- In Kenya, clinical governance gained prominence in 2018 following a grave incident at Kenyatta National Hospital. In this incident, a patient underwent head surgery only for surgeons to realize mid-procedure that they were operating on the wrong patient. This stark failure in protocol underscored the critical need for robust clinical governance practices to mitigate such errors, which sadly occur all too frequently in both public and private hospital and healthcare settings.
- The concept of clinical governance is deeply intertwined with corporate governance in healthcare. While corporate governance focuses on the overall oversighting, directionand control of an organization, clinical governance is focused on ensuring continuous improvement in quality of service and upholding the highest standards of care. Traditionally, the responsibilities of corporate governance often overshadow those of clinical governance, leading to a fragmented governance model that hinders operational efficiency and increases the risk of errors.
Integrated Governance
- To address this disparity, the concept of Integrated Governance has emerged, emphasizing the convergence of corporate and clinical governance for more cohesive oversight. At its core, clinical governance serves as a vital pillar of hospital administration, entrusted with safeguarding patient safety, provider well-being, and the delivery of exceptional clinical care.
- Integrated governance denotes the critical juncture where corporate governance and clinical governance converge to create synergy within a hospital governance system. Unfortunately, in many instances, these two governance domains operate in silos, with little to no communication or collaboration between them. This lack of alignment poses a significant risk to organizational reputation and sustainability.
- Despite sharing similar objectives, corporate and clinical governance often exist unaware of each other’s presence within the organization. This discordance not only undermines the coherence of governance structures but also jeopardizes the ability of healthcare institutions to fulfill their mission and mandate effectively. By fostering integration between corporate and clinical governance, organizations can harness the collective strengths of both domains to enhance overall performance and ensure the delivery of high-quality care, enhanced organizational reputation and better than average organizational financial sustainability. At Dawaline Consulting, recognizing the importance of this specialized area, we offer comprehensive training and consultancy services tailored for boards of management and top clinical leadership within hospitals and healthcare institutions. By equipping stakeholders with the necessary knowledge and skills, we aim to foster a culture of excellence and accountability in hospital services delivery.
Corporate Governance
- Corporate governance is defined as a system of rules, practices, and processes by which an organization is directed and controlled. The training in corporate governance is targeted to people in corporate leadership in general, but our focus will be people in corporate leadership of healthcare organizations. It is not possible to divorce clinical governance from corporate governance, and when the two are brought together, the result is integrated governance.
